La sepsi è una complicanza delle infezioni che aumenta il rischio di morbilità e mortalità del paziente. La sepsi addominale è un’importante causa di morbilità e mortalità ed è frequentemente associata ad una prognosi sfavorevole, particolarmente nei pazienti ad alto rischio. Il controllo chirurgico della fonte di infezione, la terapia antibiotica e il supporto rianimatorio, sono i veri punti chiave del trattamento delle infezioni intra-addominali. Negli ultimi 20 anni, l’incidenza di infezioni intra-addominali causate da microrganismi multiresistenti è aumentata significativamente, probabilmente anche a causa di elevati livelli di esposizione agli antibiotici e per la sempre maggiore complessità delle condizioni cliniche dei pazienti trattati. Gli obiettivi del trattamento si riassumono in un supporto intensivo per il ripristino e la stabilizzazione dell’emodinamica, un adeguato supporto nutrizionale, un’attenta selezione e somministrazione di antimicrobici ed infine un precoce ed aggressivo trattamento chirurgico al fine di controllare la fonte d’infezione, detergere il peritoneo dal pus e dai tessuti necrotici, decomprimere l’addome e infine prevenire e trattare l’infezione ricorrente. La gestione del paziente con sepsi addominale non può prescindere da un approccio multidisciplinare in cui il medico del pronto soccorso, il chirurgo, il radiologo, l’anestesista, il rianimatore, l’internista e il medico di laboratorio, devono agire in maniera sinergica allo scopo di iniziare il più rapidamente possibile il corretto trattamento. Ci auguriamo che la presente monografia si riveli una pubblicazione snella, di facile consultazione, aggiornate e ricca di suggerimenti pratici. Vogliamo infatti presentare un modello di trattamento multidisciplinare volto al trattamento dei pazienti con sepsi addominale, soprattutto quelli a maggior rischio di resistenze antibiotiche.
